• A completed initial report (WCB form) with the doctor’s signature.
• An incomplete initial report with the doctor’s signature and attached chart or clinic note(s) from the visit with the patient.
  *Chart and clinic notes must contain information similar to that requested in the initial report

• An outpatient summary sheet that supports the ongoing management of the claim.

• A typed or written detailed letter from a specialist addressed to either the WCB or the referring health care provider in response to a referral to the specialist.

• An administrative fee to print/​photocopy test results, chart notes, diagnostic imaging, etc. upon the request of the WCB.
  *If the claiming physician is required to be involved, tariff 0017 is eligible to be claimed in addition.

• Claimable for physician time required to produce chart notes at the request of the WCB.
• Eligible Physician time includes, but is not limited to time related to retrieval, production, review, redaction, and summarization of medical information relevant to the WCB claim.
• Claimable for each full 15 min period and each additional 15 min period or major portion thereof (major portion thereof equals more than half of the 15 mins, or more than 7.5 minutes)

• Payable on visits when a doctor’s progress or opioid management report is not submitted for that visit.

*follow-up visits

• Claimable when a completed opioid management initial report or opioid management progress report, with the physician’s signature is submitted.

• Claimable for telephone communications with the WCB in regards to a WCB claim.
• The communication must be initiated by the WCB or, with prior approval of the WCB, initiated by the claiming physician.
• Communications may include returning missed calls.

• Claimable for in person meetings with WCB Representatives at the request of the WCB.
• Minimum of 1 hour to be claimed.
• Preparation time and direct travel time may be claimed.
• A physician may claim a different hourly rate, where mutually agreed prior to meeting with the WCB.

• Claimable only when WCB initiates a request for expedited service.
• Timeframes for expedited consultations by the primary surgeon shall count from the date the request is issued by WCB.

Physicians should monitor if discrepancies exist between the date on correspondence from WCB and the date the correspondence is received.

• Timeframes for expedited consultations by the primary anesthetist shall count from the date the primary surgeon consults the anesthetist.
• Note, anesthetists may only claim expedited consultation services for cases where the WCB requested an expedited consultation by the surgeon.

• Claimable in addition to surgical services listed in the Manitoba Physicians Manual provided to a WCB claimant.
• The surgical service must be either approved by the WCB or provided on an urgent emergent basis.
• Claimable by the surgeon(s) and anesthetist(s) involved in the case.

• Claimable in addition to surgical assistance services listed in the Manitoba Physicians Manual provided to a WCB claimant.
• The surgical service must be either approved by the WCB or provided on an urgent emergent basis.

• Claimable in addition to all services listed in the Manitoba Physicians Manual provided to a WCB claimant.
• Claimable when surgical/​anesthetic services are provided within 30 business days of the expedited surgical consultation or within 30 business days of the WCB approval to proceed with the surgery.
• Not claimable in addition surgical administrative fees, service codes 5559 or 5560.

• Claimable in addition to pain management services listed in the Manitoba Physicians Manual by specialists in anesthesia or physical medicine.
• Pain management services may include injections and surgical services.
