Shared Health Diagnostic Services has developed a standardized Requisition Form for rural/​northern facilities, which should be used effective August 13, 2025. 

The new Req should be used to request X‑Ray/​Fluoroscopy, CT, Ultrasound, and Nuclear Medicine examinations (not MRI) at facilities not affiliated with DI Central Intake, including in Southern, Northern, Interlake-Eastern, and Prairie Mountain regions. We understand this requisition has been set up by the Provincial EMR Support Services in their system.
