Position Summary
The Family Physician at the Access Centre provides high-quality, patient-centred primary care services to individuals and families. The physician works collaboratively within a multidisciplinary team to deliver comprehensive, coordinated, and timely care, ensuring optimal health outcomes for patients.
Key Responsibilities
- Provide comprehensive primary care services, including assessment, diagnosis, treatment, and ongoing management of acute and chronic medical conditions.
- Deliver preventive care, health promotion, and patient education in accordance with evidence-based guidelines.
- Collaborate closely with a multidisciplinary team, including Occupational Therapists (OT), Physiotherapists (PT), Social Workers (SW), and counsellors, to address patients’ medical, functional, and psychosocial needs.
- Participate in care planning, case conferences, and coordinated care initiatives to support integrated service delivery.
- Refer patients to appropriate internal and external services as required.
- Maintain accurate, timely, and complete medical documentation in the electronic medical record.
- Contribute to a supportive, respectful, and collaborative work environment.
- Participate in quality improvement activities and adhere to organizational policies and standards of practice.
