While we continue our work to eliminate sick notes in Manitoba, we are turning our attention to other common third party forms from employers and insurance companies, such as accommodation requests, functional ability assessments and safe return-to-work documentation. We know these forms can vary wildly in format and complexity, and we want to make your life simpler. 

We are planning a one-time meeting next month with physicians to dig into this, with an honorarium available. 

We’ll be asking what your experience has been like. Which employer forms are clear and helpful? Which ones leave you frustrated? What would your ideal, standardized forms include — and what should they avoid?
