Tariff 8520 Hospital Care — per day can be claimed for each day of a patient’s hospital stay, regardless of whether the physician visits the patient or the length of the patient’s stay. Tariff 8520 is payable for panelled patients in hospital awaiting placement in a personal care home. 

Tariff 8511 Chronic Care is a per-visit tariff that is to be claimed for routine visits to personal care home patients and visits to patients in an Extended Treatment Unit of a hospital (see Rules of Application 16 and 18). 

Some facilities house acute care patients, ​“extended treatment unit” patients, and personal care home patients, so if you are uncertain which category a particular patient falls into, contact your regional health authority.
