Claim Tariff 8520 Hospital Care – per day for each day of a patient’s hospital stay, regardless of whether the physician visits the patient and regardless of the length of the patient’s stay.

Tariff 8520 is payable with respect to paneled patients in hospital awaiting placement in a personal care home.

Tariff 8511 Chronic Care is a per visit tariff which is to be claimed for routine visits to personal care home patients and for visits to patients in an Extended Treatment Unit of a hospital (see Rules of Application 16 and 18).

Some facilities house acute care patients, ​“extended treatment unit” patients, as well as personal care home patients. If you are uncertain of the category a particular patient falls into, contact your regional health authority.
