Coverage is subject to medical and financial underwriting.

Critical Illness Insurance
Lump-sum cash payment up to $500,000 for member and spouse (medically underwritten) or up to $60,000 on a guaranteed basis
Coverage for 24 illnesses for member, spouse and dependent children
Dependent children eligible for up for $20,000 with no medical questionnaire
Coverage ends at age 70 for member and spouse
