Employment and Social Development Canada (ESDC) reimburses physicians for completing Canada Pension Plan (CPP) disability forms. 

CPP Disability will pay up to $85 to physicians for completing the application medical report.

Physicians need to mail their invoice to the Service Canada office address noted on the initial form. Payment will be made by cheque and mailed to the address noted on the invoice. 

CPP Disability may ask physicians to provide supplementary information in the form of a narrative report and pay from $50 to $150, depending on the report’s complexity and length. Additional information can be located here.


Physicians may charge a higher fee than that established by ESDC and bill patients for the difference between their fee and the above fee. However, you should communicate with the patient in advance to avoid confusion.
