Employment and Social Development Canada (ESDC) reimburses physicians for the time spent completing Canada Pension Plan (CPP) disability forms. CPP Disability will pay up to $85 to physicians for completing the application medical report.
Physicians need to mail their invoice to the Service Canada office address noted on the initial form. Payment will be made by cheque and mailed to the address noted on the invoice.
CPP Disability may ask physicians to provide supplementary information in the form of a narrative report and will pay from $50 to $150, depending on the complexity and length of the report.

Note: Physicians may charge a higher fee than that established by ESDC and may bill patients for the difference between their fee and the above fee. Communicate with the patient in advance to avoid confusion.
